Equipment Inspection Forms
What to document when inspecting safety and rental equipment
Documenting equipment inspections before each use protects your crew from using defective equipment and protects your business from liability. Here's what to include for the most common equipment types.
1Full-Body Harness Inspection
- Webbing: no cuts, fraying, burns, or UV damage
- D-ring: no cracks, distortion, or corrosion
- Buckles: no cracks, corrosion, or rough movement
- Stitching: no broken or loose stitching
- Labels: legible, showing manufacturer and date
- Previous fall: harness must be retired after any fall arrest
2Ladder Inspection
- All rungs present and free of damage
- Side rails: no cracks or bends
- Feet and non-slip pads intact
- Extension mechanism works smoothly (extension ladders)
- Weight rating label visible
- No grease, oil, or mud on contact surfaces
3Equipter Inspection (Before Each Use)
- Hydraulic fluid level
- Tire pressure and condition
- Control operation: lift, travel, dump
- Battery charge level
- Dump box latch and release function
- Safety decals readable
- Any leaks or unusual sounds during test cycle
4Trash Rocket / Chute Inspection
- All sections connected and secured
- Ground base stable and level
- Entry gates/flaps functioning properly
- No holes or tears in chute material
- Container beneath positioned correctly
- Debris containment net (if used) secured
5Power Tools (Nail Guns, Saws)
- Safety mechanism functional
- Trigger guard intact
- No damaged cords or hoses
- Fastener magazine clear and functional
- Exhaust direction appropriate for use
6Log Entry Fields
- Date of inspection
- Equipment type and identifier
- Inspector name and signature
- Condition: pass / fail / needs service
- Action taken if defective
- Out-of-service tag applied (if needed)
💡 Pro Tip
Any equipment that fails inspection must be immediately tagged out of service and physically removed from the site. Do not allow workers to continue using equipment that has been flagged.
